Since Community Medical and Dental Care opened, our patients have depended on us to provide top quality Primary Care.

Part of our mission is keeping our patients healthy.  This includes performing regular health screenings as well as keeping annual immunizations up to date.  As a pillar of the medical community, we offer comprehensive treatment for a full range of ailments, diagnostic testing and integration with specialists where needed.  From age 22 and up our primary care professionals deliver personalized and customized care to all of our patients..

PRIMARY CARE TEAM

JAMES ISRAEL, MD

Dr. James Israel is the longest-employed physician with CMADC, having joined us when our clinic opened in 1993. He completed his medical education at SUNY Downstate Medical Center in 1970; his Residency at Maimonides Medical Center in 1973; and his Fellowship in Gastroenterology at the Albert Einstein College of Medicine in 1975. He continues to be one of our most popular physicians, due to his empathetic bedside manner and the time he takes with each patient.

DEALTRE LOLO, MD

Hailing from Haiti, where he completed his medical education and training from the Faculty of Medicine of the State University of Haiti in 1991, Dr. Lolo migrated to the United States and took on a position at Metropolitan Hospital as a Hospitalist, and currently holds positions as an Assistant Clinical Professor of Medicine and as Chairman of the hospital’s Utilization Review Committee. Dr. Lolo joined CMADC in 2007.

BRIAN BLITZ, PA-C

Brian Blitz received his medical training from Long Island University, The Brooklyn Hospital in conjunction with Downstate Medical School. For several years he worked in community health, primary care, and emergency medicine settings. In addition, Brian was employed at Beth Israel Medical Center, NYU Hospital for Joint Diseases, and Charter Behavioral Hospital for a number of years, before joining CMADC in 2009. Brian Blitz has extensive experience in medical publishing and professional medical education initiatives including work with the McGraw-Hill Companies and CMP Media. He is a Past Vice President of the New York State Society of Physician Assistants, Adjunct Clinical Assistant Professor at the Long Island University PA Program and a board certified and re-certified Physician Assistant.

LAWRENCE ROSS, MD

Dr. Lawrence Ross graduated from Albert Einstein College of Medicine, Yeshiva University in 1972.  He completed his residency in Internal Medicine and served as Chief Medical Resident at Jacobi & Einstein hospitals.  Dr. Ross completed a fellowship in Nephrology and is Board Certified in both Internal Medicine and Nephrology.  He has practiced and taught at Jacobi & Einstein hospitals from 1970 - 2020.  He joined Monsey Health Center in 2020 and sees patients in the Adult Medicine Department.

AVIVA SCHUMAN, MD

Dr. Aviva Schuman completed her medical education at the Technion Israel Institute of Technology in Haifa, Israel in 2010. She then went on to complete her internal medicine residency at Maimonides Medical Center in Brooklyn, NY. 

 

Dr. Schuman joined the CMADC team in 2023 and comes with 10 years of experience in adult primary care. She is knowledgeable in a range of subspecialties including: cardiology, neurology, women’s health, and behavioral health.

Dr. Schuman has an excellent bedside manner and is a compassionate doctor who strives to provide excellent medical care to all of her patients.

SCOTT SILVER, MD

Dr. Scott Silver received his medical degree from Thomas Jefferson Medical College in Philadelphia, PA.  Dr. Silver has a subspecialty in Geriatric Medicine and provides care to Adults at Monsey Health Center.  Dr. Silver has practiced as a hospitalist and Primary Care Physician in California prior to joining Monsey Health Center.  His is affiliated with Holy Name Hospital in Teaneck, NJ

SARA BURNBAUM, NP

Sarah is Board Certified.  She graduated from Dominican College with a master’s degree in advanced nursing, with a specialty in Family Nurse Practice.  Sarah’s goal is always to care for patients of all cultures with respect and compassion; to maintain a level of health and well being that enhances the patient’s lives.

Sarah’s specialty is the intellectually and developmentally disabled population.  Working closely with families, group homes and staff.  Sarah’s work with her patients continues beyond the office visit.  Personal care and devotion to patients is always the goal.
